On-Goal offers enrolled members a fully personalized approach to gaining control of their diabetes by making adjustments to their daily lifestyle behaviors. Personalized care and ongoing education are the keys to improving diabetes health, member compliance and may contribute to a medical costs decrease by up to 10% – based on reduced hospitalizations and ER visits.
Based on your specific needs, this program can be customized to focus on: diabetes management, health and wellness management, medication fulfillment and compliance. As an employer, this program can include one or all of the focuses to ensure you and your members needs are met.
The On-Goal program is led by a team of Licensed, Registered Dietitians that work with each individual to design a personal care path including setting goals. Members can expect to:
- Be assigned a personal Registered Dietitian
- Have availability to communicate via phone or email
- Receive education on diet, exercise, and general guidance regarding condition
- Follow up appointments hold accountability
- Achievements will be celebrated together
